A popular term for a patient management-related skirmish between 2 physicians or services that occurs in writing on the pages of a patient’s chart, with attacks and ripostes in the form of progress notes, with each party questioning the other’s justification for performing a particular procedure

In a chart war, one “combatant” may impugn another’s intelligence at least, and competence at most, cutting-edge medical literature is quoted freely, and the language used may be arcane and oblique. There is no justification for chart wars, regardless of how elegant the thrusts and parries, the chart is a document that can be quoted in a court of law
