“…the disease or injury that initiated the train of morbid events leading directly to death, or the circumstances or violence that produced the fatal injury”.

As an example, fulminant acute peritonitis is the immediate cause of death, but it is the end result of a shotgun blast to the abdomen, which is the underlying–aka primary or proximate–cause of death.
