There’s been a fair amount of mental masturbation as to the exact origin of the phrase. The Hippocratic Oath includes the promise “to abstain from doing harm”, which would have been good enough für fast jeder Mensch, but the nitpicking contingent had to put in its two cents and cried, au contraire and thus the polemic rages on…yawn.
