first bite syndrome
Definition A condition characterised by facial pain with severe cramping or spasm in the parotid region coinciding with the first bite of each meal which diminishes over the next several bites.
Comment First bite pain is a potential sequela of surgery of the infratemporal fossa, parapharyngeal space, and/or deep lobe of the parotid gland, and occurs in 49% of patients undergoing sympathetic chain sacrifice, 22% of patients undergoing parapharyngeal space dissection, 38% of patients undergoing isolated deep lobe parotid resection, and in less than 1% of patients undergoing total parotidectomy. No treatment consistently provides effective symptomatic relief.
