A rare, extreme form of paraoesophageal hiatal hernia, in which the entire stomach is in the thoracic cavity, having rotated on its organo-axial axis or cardiopyloric line, resulting in reversed positions of the lesser and greater curvatures of the stomach

Clinical findings Chronic occult blood loss with hypochromic anaemia. In infants, the typical findings include recurrent vomiting and distended epigastrium. 

Imaging Globular shadow with an air-fluid level superimposed on the cardiac shadow, which is seen in a plain chest film 

Management Reduction, gastropexy
