Gustilo classification

Gustilo classification is a system for classifying severity of open tibial fractures, which forms the basis for deciding whether to reconstruct or amputate the leg.
Gustilo classificiation grades
I Open fracture, clean wound, <1 cm in length
II Open fracture, wound > 1 cm in length without major soft-tissue damage
III Open fracture with extensive soft-tissue damage
• IIIA Adequate periosteal coverage of the fractured bone
• IIIB Extensive soft-tissue loss, periosteal stripping and bone damage
• IIIC Arterial injury requiring repair, irrespective of soft-tissue injury
