Definition A term with both nominative (the event–i.e., the hand over) and verbal (to hand over) status, for the passing of one or more patients from the care of one carer–e.g., doctor or nurse, to another at the end of a shift or when the patient is to be transferred to another service. Proper hand over of acutely unwell and at-risk patients to responsible persons on the next shift reduces error and deals with critical issues in a timely manner.

Synonym Handoff

These two terms are used interchangeably, with handover seeming preferable to handoff.
